Not to get too technical, but it depends on three things. Down, distance and field position. Not all situations are the same. And five yards does make a difference if you are not a good passing team or your punter stinks. Many a game has been won and lost by field position. The problem is not in taking the time out. The problem is not being ready for the situation when it comes up. That's what practice is for.

I vacillate on this one. But, in a game where a score is the deciding factor, I find the timeout to be more important than the yardage.
